The ADEXUSDx® Acetaminophen Test is an enzymatic assay used for the qualitative detection of acetaminophen above an established cut-off in human whole blood, plasma or serum samples as an aid for health care professionals in the diagnosis of acetaminophen overdose.

Acetaminophen is one of the most commonly used over-the-counter pain relievers and fever reducers. When ingested in therapeutic doses, the majority of acetaminophen is conjugated in the liver and excreted in the urine. Acetaminophen overdose is a common clinical problem. Ingestion of toxic amounts of acetaminophen causes more of the drug to be metabolized by the CYP2E1 isoenzyme of cytochrome P450 to N-acetyl-p-benzoquinone imine, a highly reactive intermediate. Failure to detoxify or clear this metabolite leads to fulminant hepatic failure.

The ADEXUSDx® Acetaminophen Test is a rapid enzymatic assay that needs only a drop of whole blood, plasma or serum. A sample is dispensed to the sample application zone to fill the receiving channel. When enough sample is in the receiving channel, the sample flows by capillary action into a dry porous test strip composed of a membrane array. The sample passes through the plasma separating membrane where arylacylamidase hydrolyzes acetaminophen to form p-aminophenol. There is a reaction of p-aminophenol with impregnated 8-quinolinol sulfate that results in a coloured indophenol.
